Each account must have a unique email address associated with it. Please contact us if you need multiple accounts with the same email address (i.e. related family members).
Please complete the client registration form below and you will be provided with a username and password for the client portal. Once you have a username and password, you will be able to login. If you are not yet a client and would like to schedule a free consultation you can do so via the calendar once you log in. If you are a client, the portal will be utilized to complete required paperwork, upload your photo ID, for secure email communication, and for your telehealth appointments.
